The disposable FMA is our enhanced version of the French Army Medical Department's Medical Forward sheet. Gone are the days of using A4 sheets from office printers or low-cost supplies that are unusable without a support; this FMA allows you to write without any backing, with most markers, and in an indelible manner (non-reusable).

Its improved design enables quicker use: the fillable areas are immediately visible to the user, providing a significant time-saving advantage in first aid situations.

At PHOENIX EQUIPEMENT, we understand that these forms are used under intense stress. That’s why they were developed with qualified TCCC instructors to address regulatory and technical aspects. For readability and comprehension, many human factors, such as vision, brain function under stress, and clarity for dyslexic users, have been incorporated.

Our FMAs are therefore perfectly suited for emergency situations and can be secured to the treated person using ELASTIK™, CROKYD™, or your own methods (shock cord, paracord, string, etc.) thanks to the integrated perforation.
